FAQs
Will Insurance or Medicare cover the membership fee?
Medicare and private insurance does not cover the membership fee. We as a practice will not submit claims to insurance for coverage while on a membership plan.
What is your policy on cancellation and No Shows?
When you schedule an appointment with us we set aside enough time to provide you with the highest quality care.
Should you need to cancel or reschedule an appointment please contact our office as soon as possible. This gives us time to schedule other patients who may be waiting for an appointment.
Cancellation requests with less than 24 hours notice will be charged a $25 cancellation fee.
No shows for confirmed appointments will be charged a $25 no show fee.
For New patient consultations/non member new patient appointment, a NO SHOW is a charge and automatic ineligibility for establishing care.
What is your policy on medication refills?
Refill requests will be processed as time permits during business hours. Please plan accordingly.
Refills for controlled substances require a mandatory office visit as regulated by the Florida Department of Health.
When the membership ends, a one time courtesy 30 day supply with NO refills.
What is your policy on pain medication?
Due to changes in federal and state legislation regarding narcotic pain medications, we do not routinely prescribe narcotics from this office nor do we manage chronic pain.
Are there any additional fees?
There is a $25 administrative fee for any lengthy, time-consuming forms (for example, disability forms) that we may be asked to complete for non-membership patients.
There is a $25 administrative fee for copying, faxing, etc of medical records for non-membership patients who wish to transfer their records to another practice.
These fees are waived for membership patients.
Can I use my FSA or HSA to pay for the annual membership?
Our services may qualify for reimbursement. We recommend you contact your FSA or HSA benefits administrator to confirm.
Do you accept HMO, Tricare, or Medicaid?
We are not Medicaid, HMO, or Tricare providers, and our membership model is not compatible with these plans. If you have this type of insurance, only your assigned primary care physician may make referrals and order tests for you.
Do I pay for each visit?
No, office visits are included in the membership fee.
Can I be seen just once or for an Annual visit?
With this program, we aim to develop lifelong relationships with our members and are committed to offering personalized primary care. We believe our success — member satisfaction and professional fulfillment — is derived from a care model that is built on close, personal relationships with our members. As a result, we do not offer one-time, fee-for-service medical care.

If you are looking for primary care and do not have insurance/high deductible/wanting to get more involved with the genetic testing aspect this fits such needs.
We do encourage having emergency insurance if using the membership for primary care needs.

For your convenience, we can arrange to have draw your labs in our office. Please note, however, that lab work is not included in membership. You would use your insurance for lab work; and coverage is subject to your individual insurance plan. Lab bills incurred are the sole responsibility of the member.
What if I'm hospitalized?

We will collaborate with your admitting doctor to coordinate care. While we will advocate on your behalf, it is important to know that hospital providers will be the primary administrators of your care. If you are admitted to one of the local hospitals where we have an established relationship — HCA Oak Hill Hospital, Tampa General Hospital in Hernando County – we will make every effort to see you during your hospital stay. Bills accrued during a hospital stay are the sole responsibility of the member.

For our patients that use insurance for appointments, we most certainly will offer the same modalities at cost.
If interested in the Novos Longevity Kit, this will need to be purchased through our practice and overseen by a physician with follow up appointments on a monthly basis that will be covered by your personal insurance.
